When it comes to your third molars, the question isn't always if they'll come in — it's what they'll do when they do. Not all wisdom teeth require extraction, but many do end up causing issues over time. At Coyne Oral Surgery in Franklin, TN, our team helps patients understand whether wisdom tooth extraction is necessary. While some people have enough space in their jaws for these molars to come in without problems, others experience pain, crowding, or infection. Knowing the signs to look for can help you make a confident decision with your oral surgeon.

What's the purpose of wisdom teeth?

Most people simply don't have enough room in their mouths for wisdom teeth to grow in comfortably. These third molars usually appear in the late teens or early twenties, and while they can erupt without issue, that's not always the case. When there isn't enough space, they can become impacted wisdom teeth, staying trapped under the gums or only partially erupted. This can lead to discomfort, difficulty cleaning the area, or pressure on neighboring teeth. Left unaddressed, these issues can turn into bigger oral health concerns.

How do I know if my wisdom teeth are causing problems?

There are a few clear signs you need your wisdom teeth removed, and they shouldn't be ignored. If you're experiencing any of the following, it's time to talk to an oral surgeon:

  • Jaw stiffness and wisdom teeth that make it hard to chew or open your mouth fully
  • Wisdom teeth pushing other teeth, leading to shifting or pain in your bite
  • Infected wisdom tooth symptoms, like swelling, tenderness, or bad taste in your mouth
  • Headaches, earaches, or sinus issues connected to pressure from impacted molars
  • Pain that comes and goes, often worsened by chewing

These symptoms can sometimes start subtly and gradually worsen. That's why regular dental checkups and imaging are essential for catching issues early.

What happens if wisdom teeth are not removed?

Choosing to skip oral surgery for problematic wisdom teeth might sound easier, but it can come with long-term consequences. If wisdom teeth are not removed, you could face damage to nearby teeth, the development of cysts or infections, or even misalignment of your bite. In some cases, wisdom teeth crowding can undo years of orthodontic treatment. While wisdom teeth removal vs leaving them in is a personal decision, it's important to weigh the potential for ongoing pain or complications if they stay put.

Do all wisdom teeth need to be removed?

Not necessarily, but it depends on how your teeth develop. If they're fully erupted, properly aligned, and easy to clean, removal might not be needed. However, for many patients, the risks of keeping them outweigh the benefits. Our team at Coyne Oral Surgery can use diagnostic tools, like digital x-rays, to determine if wisdom teeth are causing problems and recommend whether extraction is the path forward.

Had my wisdom teeth extracted with Dr. Coyne around a year ago today. They were impacted and she did a great job. My husband is now getting his extracted this week from her as well, and I know he and others will be in good hands if they choose her and her team. Her surgical assistant is top tier, talked with me prior to sedation due to my fears and severe dental anxiety. They were patient and so kind to me, you don’t find many places that treat and care for you more than just a patient in their office. 🦷 💜 Even after the procedure I asked for a picture with Dr. Coyne 🙂

I went in for a cracked tooth extraction and implant. While waiting for that procedure, the staff reviewed my x-rays and recommended taking the wisdom teeth and doing another implant for a tooth extraction from about 5 years back. My wisdom teeth were causing crowding and they guided me to an excellent decision. Since I was there already and was going to be sedated anyway, why not remedy the wisdom teeth issue and do both implants. I didn't feel pressured in any way. It was a great suggestion and I have absolutely no regrets. It was a lot of work in one sitting, but these folks are elite. I'm twelve days post procedure at the time I'm writing this and recovery is going just fine. I highly recommend Coyne.
